IBD Health Diary uses relevant Patient Reported Outcome Measures (PROM's) to help you assess the extent and course of your disease. The selected PROM's are especially developed to help an IBD patient assess their disease using relevant parameters. By logging PROM's frequently, it can contribute to a more constructive discussion with your Health Care Professional about your disease and your current treatment. IBD Health Diary can be used to record symptoms and follow the course of your disease, list medication to be taken and receive notifications when your next dose is due. The overview of your disease development and current treatment can be shared with your Health Care Professional via an encrypted email. The IBD Health Diary will also include a Knowledge Base section that includes updated information about IBD and all relevant aspects thereof.
