What is Nursing care plans?

A Nursing care plan contains information about the patient's diagnosis, treatment goals, specific care instructions (including observations needed and actions to be taken), and an evaluation plan.

The plan will be updated with any changes and new information as the patient's hospitalization progresses. In fact, most hospitals require nursing staff to update care plans during and after each shift.

What is the purpose of a Nursing care plan?

Here is the purpose and importance of developing a care plan:

✅ Identify the role of the nurse: It helps to recognize the unique role of nurses in caring for a patient's overall health and well-being without having to rely solely on a doctor's instructions or intervention.

✅ Offers tips on personal customer service: It enables caregivers to think critically about each client and design interventions that are directly targeted to the individual.

✅ Continuity of Care: Nurses from different shifts or departments can use this data to care for patients with the same quality and type of surgery, allowing patients to get the most out of their treatment.

✅ Documentation: It should detail what observations are to be made, what care is provided, and what direction the client or family member needs.

Types of Nursing Care Plans

There are four main types of care plans.

💖 Informal: A care plan that exists in the nurse's mind and consists of actions that the nurse chooses to perform during a shift.

🧩 Formal: This is a written or computerized plan for organizing and coordinating a patient's care information and care plan.

✨ Standardization: Provides care to patient groups with daily needs.

🎉 Personalized: A care plan tailored to a patient's specific needs.
